In order to register as a new Practitioner, please complete the registration form below. All fields with * must be completed.
Please note in order to be accepted as a Practitioner (Wholesale Account) of Nature-Provides, you must meet and adhere to the following Terms and Conditions:
- Be a current and licensed Practitioner in good standing in your field while being actively registered with Nature-Provides.
- Upload your credentials in this form as requested.
- In order to maintain Practitioner status, and therefore wholesale pricing, you must purchase a minimum of $300 per quarter, or a minimum of $1200 per annum.
Refer to the Practitioner Terms & Conditions document for more details.
Should you not meet these minimum requirements, you can opt to be an Affiliate with us for a commission. You can become an affiliate here.
Once you are registered as a Practitioner, you can simply send your patients directly to our website at www.nature-provides.com for purchase. Your patients simply register here https://nature-provides.com/patient-registration/ and we will issue you a quarterly rebate. Currently there is no charge for drop-shipping to your patients.
Quarterly discounts are paid directly to you - provided your completed W9 and bank ACH/EFT information containing routing# and account information is on file. Note we no longer issue manual checks.
